Trospium Chloride
Trospium chloride is an antimuscarinic, anticholinergic agent. It works by blocking muscarinic receptors on the bladder's detrusor muscle, reducing bladder contractions and the sudden urge to urinate. Because it is a quaternary ammonium compound, it does not readily cross the blood-brain barrier, which may result in fewer central nervous system side effects compared to some other anticholinergic medications.
Dosage by Body Weight
Adults: The standard immediate-release dose is 20 mg taken twice daily on an empty stomach (at least one hour before meals). For the extended-release formulation, the standard dose is 60 mg taken once daily in the morning with water, at least one hour before a meal. For patients with severe renal impairment (creatinine clearance less than 30 mL/min), the immediate-release dose should be reduced to 20 mg once daily at bedtime, and the extended-release formulation is not recommended. Safety and effectiveness in pediatric patients have not been established.
